Twenty-first century shiphandling introduced a condition no earlier generation of mariners faced: a constant flow of information and communication through handheld devices, computers, navigation systems and their alarms. ECDIS, PPUs, laptops, navcoms and mobile phones — and the reliance built on them — created what this chapter calls a perfect storm of electronic distraction. The shiphandler's task is no longer only to manage the ship; it is to manage the flood of inputs competing for attention while the ship is being handled.

The chapter develops one practical chain of ideas: distractions degrade performance and breed error, so the mariner must first protect proper lookout, then learn to triage incoming information by urgency, and finally know how to behave after a mistake so the first error does not cascade into an accident. Modern technology is treated as superb and safety-enhancing — but never a substitute for looking up.

There is a reason ships' bridges still have windows instead of monitors.

2. The Distraction Problem

2.1 Alarm Distraction and Over-Reliance

"Alarm distraction" has become so invasive that a junior officer will leave the EOT mid-command to attend to an alarm: the handler orders "Stop engine," an alarm sounds at the same instant, and the watch officer services the alarm before stopping the engine. Over-reliance on any single electronic aid is itself a serious distraction. PPUs have greatly increased the safe movement of vessels, yet they must not divert attention from using all available means of navigation. A mariner looking down at a portable device easily loses track of the passing minutes.

Distraction is a performance-influencing factor: like time pressure, it can help or hurt performance, and it can lead directly to human error. The danger multiplies — often exponentially — when distraction coincides with other factors such as complex or unfamiliar tasks and time pressure. Every distraction competes with the time the manoeuvre needs to be completed safely. The challenge is not to ban technology but to monitor its appropriate use.

2.2 Case Study: Atlas Air Flight 3591

The Atlas Air flight 3591 crash on approach to Houston is the chapter's central illustration of distraction, fixation on a single device, and panic. The captain handed flight control to the first officer on final approach, then spent the next minutes entering data into the in-flight computer and handling air traffic control — while still responsible for monitoring the flight instruments. The first officer, trying to increase the rate of descent, pushed the air brake forward and accidentally engaged the TOGA switch near his left wrist. The aircraft began adding power to go around, but the first officer never realised the switch had been triggered.

Flying in cloud with no horizon, the first officer lost situational awareness: the thrust pushed him back in his seat and felt like a nose-up pitch, although the instruments clearly showed the aircraft was not nose-up. Not looking at the instruments, he pushed the column forward into a dive from only 6,000 ft. The captain — eyes down on his computer — recognised the steep dive too late and pulled back without verbally taking control; the first officer was still pushing the nose down. Roughly 60 seconds elapsed between the accidental TOGA engagement and impact at 400 knots.

NTSB (2019): "While the captain was setting up the approach and communicating with air traffic control, his attention was diverted from monitoring the airplane's state and verifying that the flight was proceeding as planned, which delayed his recognition of and response to the first officer's unexpected actions that placed the airplane in a dive."

The lesson transfers directly to the bridge. Had the captain kept his eyes up on the instruments during those vital minutes, he would have seen the aircraft was not pitching up and might have had just enough time to correct the fatal error. Looking down at technology, without the disciplined habit of looking up regularly, can become a career-ending distraction — or worse.

3. Proper Lookout

Because of civil and criminal liability, the use of PPUs and ECDIS is now almost universal — and the unintended consequence has been erosion of proper lookout, the expert shiphandler's visual assessment. The best technology was developed in part to augment the Colregs and was always meant to partner the professional eye, never to displace it.

Colregs Rule 5 — Look-out: "Every vessel shall at all times maintain a proper look-out by sight and hearing as well as by all available means appropriate in the prevailing circumstances and conditions so as to make a full appraisal of the situation and of the risk of collision."

The book frames the present as a transition period: in 2022 nearly 400 accidents involving self-driving vehicles occurred on North American roads. Until AI sailing technology becomes the standard, the law and prudent shiphandling demand the long-established professional visual assessment. As long as the Colregs stand and a lack of lookout is grounds for negligence and fault, the shiphandler is legally bound to maintain a proper lookout from the bow, bridge and stern. Lookout astern is frequently overlooked — the old admonition runs: "Never forget, 50 percent of everything at sea is astern of you."

Colregs Rule 7 — Risk of collision: every vessel shall use all available means appropriate to the prevailing circumstances and conditions to determine if risk of collision exists. "All available means" includes eyes-up at least as much as eyes-down.

The practical conclusion is blunt: if you are eyes-down on a PPU or ECDIS for too long underway in clear visibility and there is an accident, you will be held liable. A new generation of tech-savvy mariners, raised with full faith in technology, must not forget that proper lookout is the law regardless of the equipment carried.

Do not remain eyes-down on a PPU or ECDIS for prolonged periods underway in clear visibility. It is simultaneously a breach of Colregs Rule 5, a recognised ground for negligence and liability, and the exact failure mode behind the Atlas Air disaster. Lookout astern — easily forgotten — is part of the duty: "50 percent of everything at sea is astern of you."

4. Triage

To manage mistakes in shiphandling, first manage distractions; and to manage distractions, prioritise the incoming information. The book borrows the hospital concept of triage — the system that separates cases needing immediate attention from those that can safely wait, commonly using colour-coded tags. When more information arrives than there are resources to handle it, the shiphandler must rank inputs by urgency. It is a skill acquired with experience, but best learned and practised from the start of a career.

Triage also governs transitions. In a developing collision situation a manoeuvring solution is the top priority — until collision becomes unavoidable, at which point the top priority shifts to saving lives, environment and property. The trained shiphandler makes that switch quickly: sounding the general alarm, dropping anchors, sounding five short blasts, calling for emergency assistance and alerting other vessels. The move from good to bad is often hard to identify, especially when buried under information from radios, navigational equipment, portable devices and mobile phones — which is exactly why the prioritising habit must be learned and practised from the start of a career.

5. Post-Mistake

Actions taken after a mistake can make things worse. Under extreme pressure, and struggling to grasp what went wrong, the shiphandler should focus on the current state and on potential solutions — not begin firing off a string of unconsidered commands. If the mistake leads to an accident, that may be only the half-way point: the new mission becomes to stabilise and mitigate the accident.

The chapter names self-doubt as the specific hazard — lost confidence in one's ability that undermines decision-making at the critical moment. The antidote is objective self-reflection: Weisinger and Pawliw-Fry (2015) recommend managing the pressure of the moment by focusing on one's experience, skills and a realistic perception of the challenge and outcome. A positive perception does not mean artificially diluting the challenge; it means making an honest assessment of one's ability to meet it. Collecting oneself and finishing the task after an accident is the professional expectation.

The Tiger Woods example anchors the point: at the 2019 Players Championship he made a quadruple bogey on the 17th, then collected himself, birdied the next hole, and the following day shot a 3-under 69 — substantially mitigating the disaster. The shiphandling rule that follows: when a mistake is made, hyper-focus. Become extremely precise about the exact position of the ship and make whatever changes are necessary at once. If the first reaction does not bring things under control, start slowing everything down; once the critical moment has passed, revert to a sustainable level of professional focus.
