Quem vacila menosUsa as ferramentas naturaisthoughts, physiological response, body movement, senses — para criar um inner dialogue / reframing que mantém foco e amplia a consciência de como as ações levam a resultados.

"The best at mitigating pressure are sensitive to their body's own stress signals" — rising heart rate, fear, confusion.

Para o shiphandlerEsses sinais significam loss of situational awareness. A self-awareness então triggers a controlled, deliberate response.

O que o handler should fazer

  • Mentally slow things down (desacelerar mentalmente)
  • Breathe more slowly (respirar mais devagar → oxigênio ao cérebro)
  • Reassess (reavaliar)
  • Gather more facts — menos opinião e emoção
  • Delegate tasks (delegar)
  • Recognise the stress trigger e responder à pressão gerada
  • Focus on the next execution required (a próxima manobra p/ manter o navio seguro)

"In a pressure situation, there is only one response requirement: that you get the job done correctly… In a stressful situation, reduction of stress is the goal. In a pressure moment, success is the goal. Thinking you have to be successful all the time means you are under pressure all the time."

Reconhecer a ansiedade (1º passo)

Perguntas do autorAo sentir raiva/medo/ansiedade: "At what moment did my emotional state change?" · "What triggered my reaction?"

Com o tempo patterns emerge gatilhos pessoais identificados reconhecíveis de antemão handler forewarned, not ambushed can interrupt the fight-or-flight response com pensamento racional.

É um processo aprendido e praticado de self-awareness; toda situação estressante é chance de praticá-lo.
O casoPerda do El Faro com toda a tripulação (all hands) em 2015. Fator dominante estudado: um persistent bias.

Dia 1: Hurricane Joaquin a centenas de milhas a SE da derrota mas em marcação quase constante e distância decrescente (rota de colisão) updates do furacão deixaram isso claro nos 2 primeiros dias.

O Master — profissional consciencioso — decidiu antes de sair de Jacksonville que passaria em segurança a sul da tempestade, e não mudou de opinião até o navio ser dominado e afundar ~40 nm a NE de Acklins e Crooked Island.

NTSB (2017)

"…confirmation bias — the tendency to search for, interpret, favour and recall information in a way that confirms one's beliefs… Once the captain made his decision to continue on the planned route, he was not swayed by information that contradicted his plan, including his officers' suggestions of route changes… unwilling to change course even though forecasts led the officers to believe they would come within 22 nm of the hurricane's eye."

USCG (2017)

"The Master… failed to download the 11:00 PM BVS weather data package and failed to act on reports from the 3/M and 2/M regarding the increased severity and narrowing of the closest point of approach to Hurricane Joaquin, and the suggested course changes to the south."

A defesa: fact-check

  • Fact-check bons sentimentos e planos vigentes como se faz com o medo: o sentimento agradável está realmente justificado? A nova informação justifica alterar o plano?
  • Confidence pode ser overconfidence.
  • Guarda mais eficaz: assumir que bias/complacency podem estar acontecendo comigo agora e eu não percebo — a transição de bom→ruim é difícil de ver e, quando vista, muitas vezes tarde demais.

Marinheiros antigos: manter uma weather anchor ready to let go e vigiar um falling barometer in good weather.

Feitos os fatos e o go/no-go → duas filosofias

Philosophy 1 — focus on riskPhilosophy 2 — focus on skill
Muito vento/nevoeiro/tráfego? Delay or cancel. Não se pôr em posição que exija exceptional skill: dirigir paralelo ao cais a 2–3 beam-widths e empurrar lateralmente. Skill excepcional não é necessária.Exceptional skill reduces risk em condições de alto risco. Risco é inevitável; o debate é reduzi-lo, não removê-lo; liability e safety exigem toda medida de mitigação.
Não são iguais numa crise. Se vento/corrente/nevoeiro/falha mecânica chegam após a manobra começar, o handler é forçado a operar nelas — êxito só é possível com training e experience prévios nessas condições.
Catch-22: quem só evitou manobras de alto risco não tem biblioteca de experiência para a extremidade. Focar só na evitação, negligenciando o high-risk training, na verdade aumenta o risco. Risco não se elimina, só se gerencia; e como o percebemos determina como o gerenciamos.
Definição (human factors)Safety-critical task analysis examines the tasks — ex.: berthing/unberthing — where human error could cause or fail to avoid a serious incident.

Conditions that raise the likelihood of human failure

  • Tarefas complexas ou difíceis de executar; insufficient training; too little time.
  • Passos boring, repetitive or unfamiliar; controles inaccessible / hard-to-use.
  • Poor interfaces, labelling or alarms; sinais unclear.
  • Dependência de comunicação sem erro em meio a ruído.
  • Fatores ambientais ruins (lighting, line of sight); over-reliance em detectar hazards em situação dinâmica.
  • Multitasking and interruptions; fatigue, stress and preoccupation.
Objetivo: eliminar ou redesenhar a tarefa para reduzir erro e melhorar detection and recovery. E, ao longo de tudo, exceptional skill and experience further reduce risk.

National Research Council, Minding the Helm (1994): "Mariners are inherently familiar with risk, not in probabilistic terms but in practical threats to safety that must be avoided or accommodated in operations."

O diferencial humanoDadas visão e ferramentas idênticas, o que difere é a individual reaction to the inputted information — um emotional component in decision making, moldado pelo mental model de cada um, que pode ser inaccurate.
O caso15 jan. 2009: 2 min após decolar de LaGuardia, o voo bateu num bando de gansos e perdeu ambos os motores.

Capt. Sullenberger assume os controles Mayday 3½ min depois pousa o Airbus A320 no rio Hudson só poucos feridos graves.

O "Miracle on the Hudson" não foi milagre: foi produto de skill, experience e deliberate calm.

Brassey et al, Deliberate Calm (2022): "He did not panic and… did not rely on a standard playbook or protocol to give him a false sense of security. Instead, he recognized the situation, mastered his internal response, and made the difficult yet necessary decision…"

A metáfora de SullenbergerEle fizera a vida toda "small, regular deposits in the bank of experience, education and training" — de modo que, no dia, "the balance was sufficient… to make a very large withdrawal."

Como reflexão calma é possívelVia self-differentiation. Regra do pronto-socorro: the first pulse to take is your own — no navio, o Master must do the same.

Dr Paul Taylor (2023): "Differentiation of self is a psychological state in which someone can maintain their sense of self, identity, thoughts and emotions when emotionally or physically close with others… The higher the level of differentiation, the higher the ability to acknowledge these feelings without becoming misguided by them."

O oposto é corrosivo: quando a ansiedade intensifica, ela "multiplies and paralyzes… we cannot see options, the big picture of objective reality" (Steinke, 2019) — e substituímos pensamento por emoção.

Quando o incidente se desenvolve

Primeiro gripar a reação emocional — "talk yourself off the ledge" concentrar no immediate next step micromanaging the fix, not the panic evitar a "deer in the headlight" syndrome.

O fechamento prático

  • Ter um emergency manoeuvring playbook — pensar/discutir manobras de emergência antes da emergência.
  • Aprender a gerir anger, fear, anxiety and panic via self-awareness e self-differentiation de antemão.
  • Emotions are contagious e se espalham rápido, p/ o bem ou o mal.
Manter-se deliberately calm estabiliza toda a bridge team e extrai sua contribuição; a ausência de calma "opens the door to panic — and panic is a cognitive assassin". Deliberate calm nem sempre evita o acidente, mas é o melhor estado mental para resolvê-lo ou mitigá-lo.
